Community Health Check Outreach Specialist

Provide is looking to recruit an Outreach Health Check Worker to deliver the NHS health checks programme across Essex. The role involves conducting health checks at community sites, communicating results to patients and offering lifestyle advice to support GP surgeries.

You will follow best practice guidelines, signpost patients to local services, make referrals to the Essex Wellbeing service and use NHS systems to book appointments and maintain stock of health check materials.
