Recovery‑Focused Mental Health Practitioner – Community Team

Somerset NHS Foundation Trust is recruiting a Band 5 Community Mental Health Practitioner for our Community Mental Health Service based in Taunton. You will manage a defined caseload, undertake assessments, and deliver evidence-based psychosocial interventions to support adults with severe mental health difficulties.

You will work as part of a multidisciplinary team, collaborating with service users, carers and partner agencies to deliver person-centred care and positive outcomes on the recovery
