Band 6 Primary Care Mental Health Practitioner role within the West Birmingham Hub of Birmingham and Solihull Mental Health NHS Foundation Trust. The post involves working in the Neighbourhood Mental Health Team to deliver triage, assessment and outcomes-based interventions for local communities.

You will collaborate with GPs and primary care professionals in a multidisciplinary setting, driving care pathways and safeguarding compliant care as part of the BSOL transformation programme.
