Lead Hospice at Home Nurse & Care Coordinator

Longfield Hospice at Home is seeking an experienced Registered Nurse to coordinate our Hospice at Home service, ensuring patients receive high-quality end of life care in their own homes. The role combines clinical leadership, caseload management, staff supervision and partnership working to support patients, families and carers.

You will coordinate care with multidisciplinary teams, supervise staff, and contribute to service planning, governance, audits and quality improvement activities.
