Job Description:
Ashford and St. Peters Hospitals NHS Foundation Trust is recruiting for a Neonatal Community Outreach Nurse to support babies and families transitioning from the Level 3 Neonatal Unit into the community.
The role emphasises continuity of care, education and partnership working to empower families after discharge. The post is NHS AfC Band 6, with full-time or part-time options and flexible hours, based at St Peter’s Hospital in Chertsey.
