SEND Transitions Navigator

Exeter College is seeking a Transitions Link Officer to act as the vital link between schools, families and the College. You will ensure learners with EHCPs and additional learning support needs experience a smooth transition into further education.

You’ll gather and share high-quality information, work with EHCP and Learning Support teams, and support panel decisions to help learners thrive. This role emphasizes inclusion and proactive collaboration with families and professionals.
