Care Packages Planner – Hybrid Social Care

Portsmouth City Council is seeking a coordinator to arrange packages of care and placements for Adult Social Care clients with external providers, ensuring timely, high-quality service.

You will create and send contract order forms, record all orders accurately, and liaise with Adult Social Care and providers to resolve queries and amend care packages as needs change. A hybrid working pattern is in place, with a focus on customer service and attention to detail.
