About the role

You will provide 1:1 support throughout the day, helping the client remain comfortable, safe and as independent as possible at home and in the community.

The client has cognitive impairment, reduced mobility and dysphagia, with a modified diet required. They also have a PEG, which is used for hydration and medication when they are unwell or their cognition is reduced.

  • Carrying out daily skin checks and preventative skincare
  • Monitoring the PEG site and skin for signs of infection or damage
  • Providing patient, respectful and person-centred support throughout the day

Two people are required for safe transfers, with all moving and handling carried out in line with the client’s care plan and physiotherapy guidance.

  • Manual handling and mobility support
  • Cognitive impairment
