Inpatient Social Worker – Discharge & Care Planning

Oxford Health NHS Foundation Trust is seeking an experienced Social Worker to join the Buckinghamshire Social Care Team as an inpatient Social Worker across Buckinghamshire. The role involves assessing inpatients under the Care Act to support timely, safe discharges into the community.

You will join a small team of social workers based at the Whiteleaf centre, with induction and shadowing to help you settle in from day one. A full driving licence and access to a vehicle are required.
