Senior Clinical Pharmacist — Hospital-at-Home Lead

HCRG Care Group is hiring a Clinical Pharmacist (Band 8a) to join the Hospital at Home team and optimise medicines for frail patients at home. You will work with clinicians to ensure safe, cost-effective prescribing and contribute to clinical governance and service development.
