Social Worker – Leaving Care (Flexible Hours)

Sanctuary Personnel Limited is seeking a Social Worker to join the Leaving Care Team in Hackney, London. The role involves managing complex caseloads, conducting statutory assessments and developing Pathway Plans to help care-experienced young people transition to independence.

You will build strong relationships with young people, ensuring their voices shape planning and decision-making, and coordinate access to housing, education, and health services. SWE registration is required.
