Community Health Support Worker — Enablement & Home Care

NHS Highland is seeking to recruit for the South Skye Community nursing team. It is expected that the HCSW will work independently with patients in the community and within the agreed parameters of a defined care plan.

They will work as part of the multidisciplinary team, enabling people to achieve their optimal level of independence utilising a reablement/enablement programme of care. A full UK driving licence is required with access to a vehicle.
