Social Worker – Transitions to Independent Living (Devon)

Transitions Adult Social Care (TASC) Team is recruiting a full-time Social Worker to join our countywide service, with focus on planning transitions for young people aged 14–18 into adult services. The role requires strong collaboration with partner agencies and robust understanding of the Mental Capacity Act.

You will work within a supportive team and be offered a clear PCF-aligned career pathway, with progression opportunities from Grade F to Grade G as skills develop.
