Role Description The Care Coordinator is a full-time, on-site role based in Ipswich. This role is responsible for organising and overseeing care plans, coordinating services and ensuring that individuals receive appropriate and timely support. Day-to-day tasks include assessing needs,rostering liaising with healthcare professionals and community providers, and maintaining accurate records. The Care Coordinator communicates regularly with clients and their families to review progress, address concerns, and adjust services as needed. The role also involves monitoring service quality, supporting home care arrangements, and collaborating with internal teams to improve care pathways.

Qualifications

  • Strong skills in Care Coordination and Case Management, with the ability to develop, implement, and review person centred care plans.
  • Effective Communication and Customer Service skills to interact respectfully with clients, families, and multidisciplinary teams.
  • Experience or knowledge in Home Care settings, including coordinating in-home support and community-based services.
  • Organisational and time-management skills, with attention to detail in documentation and follow-up.
  • Ability to work collaboratively, use sound judgment, and respond calmly in changing or complex situations.
  • Basic proficiency with digital record-keeping systems and standard office software.
  • Relevant experience in health or social care; formal training or a qualification in care, nursing, social work, or a related field is beneficial.
  • Understanding of safeguarding, confidentiality, and person-centered care principles.
