Primary Care Pharmacist – Prescribing & Patient Care Lead

Operose Health Ltd is seeking a pharmacist to join our primary care team. You will work closely with clinicians to help improve patient care, support cost-effective prescribing decisions, and contribute to medicine reconciliation.

You will run daily telephone triage clinics, review hospital correspondence, lead the repeat prescription service, and participate in home visits and immunisations while pursuing ongoing professional development.
