Hospital Interface Social Worker — Fast-Paced, Impactful Role

Somerset Council is seeking a Hospital Social Worker for an onsite, full-time role within a fast-paced hospital interface service. You will assess risk, support independence and coordinate discharge planning with health colleagues.

The role requires a recognised social work qualification, registration with Social Work England and access to a vehicle. Rotational Saturday work and 30 days annual leave are included as part of the package.
