EHC Co-ordinator: Flexible, Impactful Casework Lead

Hammersmith and Fulham Council is seeking an EHC Coordinator to coordinate the delivery of education, health and care casework for children and young people aged 0–25, ensuring timely, legally compliant provision.

You will be the main contact for young people and families, working within a multi-disciplinary service to deliver statutory duties under the Children and Families Act 2014, with a focus on collaborative partnership.
