Senior Community Mental Health Practitioner – Home Visits

Dorset HealthCare University NHS Foundation Trust is seeking a Band 6 Community Mental Health Practitioner in Shaftesbury. The post is full time (37.5 hours, Mon-Fri 9:00-17:00) with consideration for part-time. You will join an integrated MDT and focus on care coordination, assessment, and multi-agency liaison to support recovery.

DBS check is required and funded by the Trust. Driving is essential to travel to patients in their homes and other sites within the locality.
