Senior Health & Social Care Lead – Discharge to Assess

NHS Highland is seeking a Band 4 Lead Senior Health & Social Care Worker to join the Discharge to Assess Service in Nairn & Ardersier on a permanent basis. The role will require working flexibly with a 7-day shift rotation to meet service needs and deliver patient-centred care.

The successful candidate will collaborate across multi-agency settings, uphold high professional standards, and support the caseload holder to ensure compassionate, integrated care.
