Community Nurse – Learning Disabilities Specialist

West London NHS Trust is recruiting a Band 5 RNLD to join the Ealing Community Team for People with Learning Disabilities. The post focuses on assessment, care planning and case management for adults with learning disabilities, ensuring physical health needs are addressed within a multidisciplinary framework.

You will work with psychiatry, psychology, therapy and social care colleagues to support holistic care, promote health and enable appropriate hospital admissions and discharges.
