Job Description:
Gateshead Health NHS Foundation Trust in the North East of England is seeking an experienced Frailty Nurse Practitioner to join the Frailty Service, rotating between the Acute Frailty Team and the Frailty Virtual Ward.
You will deliver advanced clinical assessment, non-medical prescribing and care planning for people living with frailty across acute and community settings, working with patients, families and the multidisciplinary team to promote independence and safer, closer-to-home care.
