Social Worker – 0-25 Disabilities (Flexible, Hybrid)

Leaders in Care is seeking an experienced Social Worker for a long-term 0-25 Disabilities role in North Wales. You will support children, young people and young adults with complex needs, with flexible and hybrid working arrangements and a manageable caseload.

You will complete assessments, care planning, safeguarding, and engage multi-agency partners. Registered with Social Work England and experience in 0-25 disabilities are essential to deliver child-centred services.
