Senior Primary Care Mental Health Practitioner – Lead in Care

Hertfordshire Partnership University NHS Foundation Trust is recruiting a Senior Primary Care Mental Health Practitioner (Band 7) to work across HaLo PCN GP surgeries in St Albans and surrounding sites. The role involves travel between sites and delivering autonomous early assessments within a multidisciplinary team.

You will triage, assess, case manage and discharge when appropriate, advise on medications, and refer to secondary care as needed.
