Child & Family Early-Help Support Coordinator

South Gloucestershire Council is seeking a Social Work Assistant to support disabled children and young people, their families and networks, providing practical help to promote inclusion and independence within the 0-25 Social Care Service.

You will chair review meetings, visit children and families in homes or schools, and work with partner agencies to coordinate tailored support and ensure positive outcomes, including EHCP contributions.
