Gracefield Health Care Limited provides residential services within the North Liverpool (Anfield) area for adults between the ages of 18 and 65 years who have a learning disability and associated mental health needs.

We are a family run company, who will provide excellent support to the successful candidate joining our services. We are an independent care provider registered with the Care Quality Commission and effective communication is a central and essential part of the delivery of our services and we pride ourselves in being a warm, welcoming, and caring team. Our number one priority being our residents.

We are currently looking to recruit full time and part time support workers for a home for up to 6 adults with Learning Disabilities. The job will be challenging but there is a good team in place at the home and the position will be extremely rewarding for the right candidate.

Key Objectives

To play a key role in actively promoting and supporting individuals to live safely and independently within the home and community. To assist in the provision of a flexible and personalised service and act as a link between Residents, Staff, Families and Professionals,

To promote good working practices by means of an awareness and understanding of the company’s policies and procedures, as well as the aims and objectives of the organisation. We also support staff within the service to carry out their role effectively by embedding values and ethos of the service.

Under supervision of the Team Leader, the job holder will provide a range of assistance with the key tasks that enable people to maintain an independent and ordinary lifestyle wherever possible.

Main Duties

This is not a comprehensive list of all the tasks which may be required of the post holder but is illustrative of the general nature and level of responsibility of the work to be undertaken.

The ideal candidate will have an ability to follow care plans and risk assessments and work positively within the team. An understanding of Person Centered Plans and a willingness to promote good communication both oral and written for individuals needs and outcomes.

Offering and providing assistance to residents, appropriate to the individual’s level of need, with all aspects of personal hygiene, administration of medication, ensuring adequate and nutritional meals are available, and to provide support to access community based activities. There will also be a requirement to attend appointments, support with finances and assisting residents to retire to bed.

Encourage the development of keyworker relationships and responsibilities. To enthusiastically follow support plans and risk assessments and contribute to the effectiveness of individuals programs in order to support our residents to live a fulfilling lifestyle.

Care and support for the residents is the first priority. however, during the course of the day there is usually adequate time to undertake agreed domestic tasks. These tasks are likely to include laundry and ironing, kitchen and bathroom hygiene, vacuuming and general cleaning of communal areas.

The ideal candidate will have current knowledge of relevant legislation and possess a suitable qualification preferably NVQ/QCF level 2. Experience of learning disability and challenging behaviour is desirable, as the needs of the adults are complex but not essential for this position as full training will be given.

Pay rate £12.71 per hour.

Enhance Criminal Disclosure for the criminal records bureau is required for this role.
