We are looking for a compassionate and enthusiastic Support Worker to join our team in the Stratton St Margaret area of Swindon. Working across supported living and outreach services, you will provide person-centred support, promoting independence, choice, dignity, and wellbeing while enabling people to live fulfilling lives in their own homes and communities.
Key Responsibilities
– Support people with daily living tasks, including cooking, cleaning, shopping, budgeting, and attending appointments.
– Provide personal care where required and support individuals to maintain their health and wellbeing.
– Administer medication in line with training and organisational policies.
– Encourage independence, choice, and community participation.
– Follow support plans, risk assessments, and positive behaviour support strategies.
– Maintain accurate records and complete necessary documentation.
– Identify and report safeguarding concerns promptly.
– Work collaboratively with colleagues, families, and professionals to achieve positive outcomes.
– Caring, compassionate, and reliable approach.
– Good communication and teamwork skills.
– Ability to work flexibly, including evenings, weekends, and bank holidays.
– Basic IT skills and ability to maintain accurate records.
– Commitment to promoting equality, inclusion, and person-centred support.
– Experience supporting people with learning disabilities, autism, mental health needs, or physical disabilities.
– Health and Social Care qualification (Level 2 or above).
– Full UK driving licence.
