Bank Band 6 Care Coordinator – Community Mental Health

Hertfordshire Partnership University NHS Foundation Trust (HPFT) is seeking a Band 6 Care Coordinator to join our Community Adult Mental Health Team in Borehamwood on a bank contract. You will manage a defined caseload, coordinate care across agencies, and deliver person-centred, recovery-focused interventions under the CPA.

You will work within our Temporary Staffing Team, supporting high-quality care while maintaining risk assessment, safeguarding, and multi-agency collaboration.
