Home-Based Community Therapy Support Worker (D2A)

Wye Valley NHS Trust in Herefordshire is seeking two full-time Band 3 Community Therapy Support Workers to join the Discharge to Assess service. You will work with Occupational Therapists and Physiotherapists to support patients to regain independence at home after hospital discharge.

You will visit clients in their own homes and care homes across the county, provide rehabilitation and reablement support, and help with equipment fitting.
