Overview

As a Clinical Pharmacist, you join a multidisciplinary primary care team to optimise medicines across GP practices within a federation. You will conduct patient-facing clinical assessments, manage polypharmacy, and lead safe prescribing initiatives. The role supports your development toward independent prescribing and offers mentoring, with opportunities to impact patient outcomes across multiple practices. This is a meaningful chance to advance medicines optimisation within a collaborative primary care network.

Responsibilities

  • Manage chronic disease workups and clinical medication reviews for polypharmacy
  • Handle repeat prescriptions, acute requests and transfer-of-care medicines reconciliation
  • Provide clinical medicines advice and support to practice staff
  • Lead medicines optimisation, quality improvement and some QOF/enhanced services
  • Facilitate integration with community and hospital pharmacy to optimise skill mix
  • Mentor and develop within a multidisciplinary team, with opportunities to become a non-medical prescriber if required
  • Support safer prescribing systems and patient access to healthcare

Key requirements

  • Undergraduate degree in pharmacy and GPhC registration
  • Independent prescribing qualification (desirable)
  • CPPE Pathway (desirable)
  • Experience of working in Primary Care (desirable)
  • DBS check and flexible to work outside core hours
  • Valid UK driving licence (desirable)
  • Current UK professional registration
  • Excellent written and oral communication
  • Strong IT and digital literacy
