Children’s Community Nurse — Leading Care at Home

Stockport NHS Foundation Trust is seeking an experienced Children’s Community Nurse (Band 6) to join the team for a part-time 30-hour post. You will work autonomously to provide high-quality nursing care to children and young people in homes and community settings, collaborating with PSDEC, wards, emergency services and general practice.

You will deliver evidence-based care, support discharge planning and contribute to service development, with opportunities to influence pathways and audit.
