Independent Community Nurse – Home Visits, Band 5

Cornwall Partnership NHS Foundation Trust is seeking two Community Nurses for the St Austell Community Nursing Team. This Band 5, permanent role offers 37.5 hours per week, with full-time and part-time options, in home and community settings.

You will assess and provide care, collaborate with patients, families and services, and travel across the county to deliver high-quality nursing support while pursuing ongoing professional development.
