Hybrid Adult Locality Social Worker — Community Impact

Leaders in Care is recruiting an Adult Locality Social Worker to join a busy, community-based service in Wales. The role offers hybrid working with only 1–2 office days per week and a competitive hourly rate.

You will complete Care Act assessments, reviews and support plans, manage complex caseloads in the community, undertake safeguarding enquiries, and work closely with families and health professionals to enable access to appropriate services and promote independence.
