Therapy-Led Reablement Support Worker

Blackburn with Darwen Council is recruiting Support Workers for a 13-bed Residential Care Home delivering short-term, rehabilitation-focused care to adults coming from hospital or at risk of admission.

You will work under the Registered Manager, providing front-line care in line with therapy-informed plans, collaborating with hospital, therapy and nursing teams, and joining a 24-hour rota including weekend shifts.
