The London Borough of Merton is seeking a dedicated and compassionate Qualified Social Worker to join our Adult’s Locality Team. This pivotal role involves working closely with vulnerable adults to assess their needs, develop care plans, and ensure they receive the support required to lead fulfilling lives.
Key responsibilities include:
- Conducting comprehensive assessments of clients’ needs and strengths.
- Developing, implementing, and reviewing care plans in collaboration with clients and their families.
- Providing ongoing support and advocacy for clients to access necessary services.
- Collaborating with multidisciplinary teams and external agencies to ensure holistic care delivery.
- Maintaining accurate records and documentation in compliance with relevant legislation and policies.
To be considered for this role, candidates must possess the following qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Proven experience in adult social work, particularly in a locality or community setting.
- Strong assessment and analytical skills, with a person-centered approach.
- Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
