Overview

As a Clinical Pharmacist at Custom House Surgery, you will support high-quality, patient-centred care within a multidisciplinary team. You will lead structured medication reviews, medicines optimisation, and safe prescribing for patients with long-term conditions, while contributing to QOF targets and service improvement. You will collaborate with GPs and other healthcare professionals to reduce health inequalities and enhance medicines management in a diverse community. Responsibilities

  • Conduct structured medication reviews and medicines optimisation for complex patients
  • Support long-term condition management to ensure safe and effective medicine use
  • Manage prescribing queries and provide expert guidance to clinicians
  • Review hospital discharge letters and reconcile medicines
  • Identify and mitigate prescribing risks and adverse drug reactions
  • Support repeat prescribing systems and conduct audits
  • Contribute to QOF and contractual targets
  • Provide patient education and promote shared decision-making
  • Collaborate with GPs, nurses, ANPs, HCAs and wider healthcare professionals
  • Participate in meetings, service development and quality improvement initiatives

Key requirements

  • Master’s Degree in Pharmacy (MPharm) or equivalent
  • Registered Pharmacist with the General Pharmaceutical Council (GPhC)
  • Independent Prescriber qualification
  • Post-registration experience in primary care or medicines optimisation
  • Knowledge of prescribing guidelines, medicines management and clinical governance
  • Experience in structured medication reviews and medicines reconciliation
  • Ability to manage long-term conditions and optimise therapy
  • Excellent communication and interpersonal skills in a multidisciplinary setting
  • Strong clinical decision-making and problem-solving skills
  • Proficiency with clinical systems and Microsoft Office, CPD mindset
  • Understanding of confidentiality, information governance, safeguarding, and equality and diversity requirements
