Hybrid: Adult Social Worker for Planned Care Backlog

Ceredigion Council is seeking an experienced Adult Social Worker to join the Planned Care Team in Aberaeron. The role involves reviewing Care and Support Plans for older adults, with hybrid working and a long-term contract through April 2027.

You’ll conduct assessments and care planning, working collaboratively with families, carers and other professionals to ensure the right support is provided. A UK registration and Enhanced DBS are required.
