Overview
You join Lakeside Healthcare as a Clinical Pharmacist working within a multi-disciplinary team to optimise medicines use in primary care. You will lead clinical medicines management, conduct structured medication reviews, and support safe prescribing across care transitions. This role supports PCN-wide service development and quality improvement, contributing to better patient outcomes and adherence to guidelines. You will work across sites and with practice teams to grow access to pharmacist-led care.
Responsibilities
- Clinical medicines management and safe prescribing across care transitions
- Remote or face-to-face structured medication reviews for polypharmacy and long-term conditions
- Provide patient-facing support to optimise medication use and adherence
- Hospital discharge medicine reconciliation and continuity of care
- Lead clinical audits and contribute to quality improvement initiatives
- Guide service development and implement medicinal components in new services
- Enhance medicines safety, focusing on high-risk groups and unplanned admissions
- Educate and provide guidance to practice staff and patients with monitoring outcomes
- Monitor and implement local and national guidelines, assist with formulary updates and NICE guidance
Key requirements
- General Pharmaceutical Council registration
- Evidence of CPD
- MPharm degree (or equivalent)
- Completion of CPPE pathway or working towards
- Independent prescriber or working towards
- Minimum 2 years post-qualification experience
- 1 year experience in general practice
- Experience using SystmOne
- UK professional registration and right to work (as per job description)
