Overview

As an Independent Prescribing Clinical Pharmacist in General Practice, you will work within a Pharmacy team to optimise medicines for patients with single or multiple conditions. You will be supported by a GP mentor and collaborate with nurses, care home staff, and other healthcare colleagues to improve safety, monitoring, and adherence. Your work spans long-term condition management, medication reviews, and care coordination across practices, homes, and community services. This role offers impact on patient outcomes, medicines safety, and service development within a multidisciplinary setting.

Responsibilities

  • See and optimise medicines for patients with chronic conditions (e.g., COPD, asthma) and review ongoing medicine need and monitoring
  • Conduct clinical medication reviews and provide prescribing/monitoring recommendations to nurses or GPs
  • Collaborate with care home staff to improve safe medicines ordering and administration
  • Attend multidisciplinary case conferences and conduct structured medication reviews in practice or home visits
  • Conduct risk stratification and monitor high-risk patients to reduce adverse outcomes
  • Support extended access clinics and contribute to new services with medicinal components
  • Provide medicines information to staff and patients and follow up on changes
  • Reconcile medicines post-discharge and ensure continuity of supply for high-risk groups
  • Develop and implement repeat prescribing policies and ensure appropriate monitoring
  • Engage in education and training of the primary care team on therapeutics and medicines optimisation
  • Analyse medicines data to inform decision making and support quality improvement
  • Ensure CQC compliance related to medicines and contribute to public health campaigns

Key requirements

  • Masters degree in Pharmacy (MPharm)
