You work flexibly across sessions and with the Primary care MDT to ensure safe, holistic care. Responsibilities
- Provide high-quality care independently to patients and families
- Deliver safe, individualized care across flexible sessions (Mon–Sat)
- Collaborate with the Primary care Multidisciplinary Team
- Contribute to holistic care plans across settings
- Conduct Home Visiting consultations as required
Key requirements
- Experience of seeing patients in a sole environment
- Minor illness management experience
- Advanced Care Practitioner Qualification or equivalent
- Non Medical Prescribing Qualification
- Evidence of continuing professional development
- Non medical IR(ME)R course
- UK registration as a health professional
- Excellent interpersonal and communication skills
- Reliability
