This is a fantastic opportunity for an experienced and motivated practitioner to work in a fast-paced, integrated environment, supporting people to achieve safe, effective and timely transitions from hospital into the most appropriate care and support setting.

What we’re looking for :

As a member of the Integrated Hospital Discharge Team, you will:

• Work collaboratively acrosshealth and social careto support effective hospital discharge.
• Work autonomously across different settings to respond flexibly to changingservice demands.
• Assess individuals’ needs and contribute to person-centred discharge and care planning.
• Advocate for individuals and ensure their needs, wishes and outcomes remain central to decision-making.
• Confidentlychallenge and professionally negotiatewith other practitioners where health and social care perspectives or proposed options differ.
• Develop new skills and broaden your experience by working across different areas of the service.
• Manage multiple priorities and competing demands while maintaining high professional standards.
• Work totight timescales, ensuring discharge processes are progressed efficiently and safely.
• Build effective working relationships with colleagues and partner agencies across health and social care.
