Wye Valley NHS Trust in Herefordshire is seeking a Band 4 Therapy Assistant Practitioner to join our Community Team. You will work with Physiotherapists, Occupational Therapists and other staff to assess and deliver rehabilitation in patients’ homes and bedded units, supporting discharge pathways.
You will assess independence, provide equipment and rehabilitative techniques, and contribute to care planning and service development while maintaining high professional standards in a busy community
