Community Nurse (Band 5) – Flexible Home Care

West London NHS Trust is recruiting for an Adult Community Nurse to provide nursing care to housebound patients within the Hounslow locality and surrounding areas. You will work with the locality nursing team to deliver services 24/7, supported by a multidisciplinary network across primary care, social services and community teams.

The role requires current NMC registration, experience in community settings and strong communication and clinical skills.
