Job Description:
West London NHS Trust is seeking a motivated Discharge Coordinator to join our Hospital Discharge Team in Twickenham. You will manage a caseload, coordinate safe, timely discharges and work with multidisciplinary teams to remove barriers to discharge.
You will lead on complex discharge planning, liaise with social care and community services, and support staff to ensure patients are discharged safely and efficiently, reducing length of stay.
