The role involves working across GP practices to assess and support older adults with dementia and frailty, collaborating with primary care teams, families and carers to promote independence and reduce hospital admissions.
The post is based primarily in GP surgeries and offers professional development, clinical supervision and a relocation allowance of up to £8,000.
