Job overview
This role helps patients, carers and healthcare professionals access the right support quickly, making a real difference to timely community care. Based within the Frailty team, you will be the first point of contact for referrals, advice requests and service enquiries, helping people get connected to the services they need.
You’ll work closely with clinical colleagues, care homes, GP practices and partner organisations to support the smooth running of the Urgent Community Response service. The role combines customer service, coordination and administration, ensuring accurate information is recorded and shared while maintaining patient confidentiality.
Main duties of the job
What you’ll do:
- Act as the first point of contact for healthcare professionals, care homes, patients and carers
- Coordinate referrals and appointments, ensuring information is processed accurately and promptly
- Maintain patient records and update electronic systems
- Provide administrative support, including meeting preparation, minute taking and diary management
- Work with colleagues to improve processes and support service delivery across the team
What we’re looking for:
- A qualification in Business Administration, Customer Service or equivalent experience
- Experience in an office, administration or customer service role
- Strong communication and organisational skills
- Good attention to detail and confidence using Microsoft Office systems
- A flexible and team-focused approach, with a commitment to excellent customer service

Qualifications
- Level 3 in Business Administration or Customer Service or equivalent knowledge and experience
- Word processing to OCR 3 or able to demonstrate appropriate level of skill.
Knowledge
- Knowledge of a range of routine administration procedures
- Knowledge of a range of software programmes including Microsoft Word, Excel, Outlook and EPR systems
- Knowledge of SystmOne functions and other clinical reporting systems
- Knowledge of clinic/appointment processes
Experience
- Previous experience of working in an office environment and carrying out general officer procedures
- Experience of SystmOne functions
- Previous NHS experience
- Previous experience of working with general public
