Senior Night Community Nurse Home Care Specialist

Procare Health Ltd is seeking a Nights Senior Community Nurse (Band 6) to deliver high-quality nursing care to patients at home during night hours across Guildford and Waverley. The role requires autonomous clinical assessment, triage, and coordination with GPs and hospices to prevent avoidable hospital admissions.

You will work 22 hours per week (2 night shifts), be registered with NMC, and hold a nurse diploma or degree with community nursing experience.
