LPFT’s adult learning disability service, provides specialist health support to people with learning disabilities who require assessment and/or treatment for their physical and mental health, including support with behaviours of concern. The adult learning disability service operates from three multi-disciplinary hubs, across Lincolnshire.

The core hours of the role are 9am-5pm Monday to Friday but there may be a need on occasions to work flexibly to support the service.
Main duties of the job
This is a community and hospital-based role providing physical healthcare support and liaison to professionals both within the Learning Disability service and partner agencies including GP practices and hospitals.
You will be responsible for ensuring a high standard of physical healthcare support is provided for service users with a Learning Disability whose complex physical healthcare need cannot be met without the support of specialist Learning Disability services.
The role involves managing a small complex caseload of patients who have a learning disability. You will work closely with the ULHT safeguarding lead in coordinating patient care providing support to the Acute liaison nurses and with primary care colleagues to support Community liaison nurses. As part of this role there is a requirement to support the validation of GP Quality Outcome Framework Learning Disability Registers annually. Part of the role will involve engaging with practices whose registers are not part of the Validation process to understand if additional support is required.
You will provide management and clinical supervision to staff within the service and provide support and training in respect of community and acute physical healthcare to the Learning Disability Service. To ensure a high standard or clinical practice and that clinical skills are updated as required.

Ensure clinical practice of members of the Learning Disability Liaison team is of a high standard and in line with best practice.
Ensure the Learning Disability Liaison team receive supervision, appraisal, and mandatory training in line with LPFT’s policies.
Contribute to, and where requested, lead on the development of the Learning Disability Service.
Coordinate, and prioritise the workload of the team and ensure that all aspects of clinical and administrative are resourced within the limitations of the available staffing resources.
Ensure that staff develop the knowledge and skills required to meet the needs of clinical delivery, identifying to the Service Manager where there may be a skills deficit or lack of access to training required.
To undertake serious incident and complaint investigations on behalf of the wider Trust as requested by the Associate Director or The Quality Lead.
Undertake Audit of the Learning Disability Service performance and compliance to national and local standards.
Implement service development and action plans within the team and ensure that ‘lessons learnt’ from investigations are embedded within the team’s practice.
Ensure that information from Service and Trust meetings are passed to the team as appropriate and that team views are represented at Service and Trust meetings.
To deputise for the Service Manager when requested to do so.
Keep up to date on current practices in mental health and learning disabilities, including and lead on departmental training programmes and professional development education.
To support recruitment within the team and across the Learning Disability Service.
To attend relevant regional and national contributing professional development activities to maintain up-to-date skills and knowledge in meeting the needs of service users.
Assist the Service Manager by monitoring performance and activity targets.
Support the development and use of systems to monitor and review the Service Provision against national and local standards as a means of initiating and implementing change and ongoing improvement in services content, quality and delivery.
Play an active role in relevant professional and service improvement groups, within the service and Trust-wide.

Qualifications
- Degree in Learning Disabilities, mental health and social care or equivalent: 1st Level Registration (NMC, HCPC)
- Evidence of specialised continued professional training in clinical practice (Master’s degree level) or able to evidence equivalent experience
- Specialist qualifications relating to physical healthcare and Learning Disability
- Leadership Training
