Jarrodean Healthcare Recruitment is supporting a Bristol-based Complex Health & Social Care and Mental Health service to appoint a Care Co-ordinator Manager. The role reports to the Group CEO and leads the centralised Care Coordination Service across the organisation.
The post requires at least two years in care coordination, plus supervisory and multi-disciplinary collaboration experience, with a strong focus on safeguarding, risk escalation and performance oversight.
