Community Health Care Support Worker

Betsi Cadwaladr University Health Board in North Wales is seeking a district nurse to support the community nursing team in Conwy Locality, delivering patient-centred care in partnership with other health professionals, patients, families and carers.

You will travel to patients’ homes, work well in a team and also independently, and contribute to modernising the service. Welsh language is desirable; English or Welsh speakers are welcome.
