Memory & Mental Health Care Coordinator

Navigo is seeking a Mental Health Practitioner to join the Care Home Liaison Team in North East Lincolnshire. You will assess, develop, implement and evaluate person-centred care plans, working closely with a multidisciplinary team to ensure high-quality mental health support for service-users.

You will liaise with GPs and carers, promote innovative approaches to care, and manage a demanding workload in a fast-paced community setting.
